An ideal employee evaluation form has to have clear and achievable goals and objectives. When the management sets easily achievable goals for the employees, it’s easy to increase performance because employees can see that the defined goals are easy to achieve. On the other hand, if set goals are difficult or complex, it can create a lack of motivation or interest in the job because employees won’t be able to get to the point that is expected by their management.
Performance evaluation should be periodic:
Most companies use employee evaluation forms once a year usually before increments or promotions but the majority of multinational companies understand that when it comes to performance appraisal, it has to be done more than once a year. We are not asking you to evaluate your employees’ performance each month but it should be done after each big project so if there is a lack of performance, it can be eliminated in the next big project.
Employee feedback is a must:

This is the part where the employees are asked to write about their strengths, weaknesses, and areas of their responsibilities that they understand can be or must be improved.
